Job Title: Senior Assistance Case Manager
Job Classification : Non-exempt (Temporary part-time); Department: Social Services
Reports to: Social Services Manager Date: 1/26/2022


Responsible for helping seniors with physical limitations and / or LEP who need assistance managing their daily life activities, concerns and issues. Individual will provide advocacy and assistance with housing, benefits, insurance and make appropriate referrals as necessary.

Essential Duties and Responsibilities:

• Help seniors find and access resources to make their daily life easier
• Advocate with landlords, utility companies, and benefit programs by collecting and preparing all needed applications and documentation
• Connect to City, state, and community-based services and provide support in navigating these systems
• Provide language interpretation and translation of documents for clients
• Provide clients with referrals to PAA, applicable services / programs and follow-up as necessary.
• Maintain written records for each client in accordance with PAA requirements
• Help clients achieve wellness and autonomy
• (Other duties may be assigned).

Supervisory Responsibilities: none

Job Relationships: Works closely with Social Services Manager

Education and/or Experience: Associate’s degree in Social Services, or related fields. Experience working with senior and immigrant community.

Qualification Requirements:

• Some experience in case management and/or counseling
• Basic knowledge of social service providers and benefits
• Demonstrate positive communication skills

Language Skills: Ability to communicate fluently in both Polish and English

Computer Skills: Basic Microsoft Office, data management skills

Physical Demands:

• General office work
• Must have valid driver’s license (some driving may be required)
